Helpful information for new members

Yes. If you want to change your health plan assignment, simply contact Health Share Customer Service at 503-416-8090 or at 888-519-3845 (TTY/TDD is 711) and ask them to help you change your plan assignment. If you know you are going to move to another county within Health Share’s service area, you can call Customer Service before you move and they will help you find care close to your new home.

Yes. When you become a Health Share member, you can choose a Primary Care Provider or we can assign one for you. This could be a doctor, a nurse practitioner or a physician’s assistant. Start with your PCP for all of your health care needs because they know you best.

All Health Share members have a PCP. Your PCP is probably a clinic or private provider in your city or close to your home. If you do not choose your PCP yourself, your PCP assignment is based on different factors, including where you live.

You can also choose providers from your dental and mental health plans, too. All of their providers are listed online, or you can call your dental plan or mental health plan, and they’ll help you find the support you need.

Your PCP works with you to help you stay as healthy as possible. Your PCP keeps track of all your basic and specialty care. If you are seeing your PCP for the first time, make an appointment as soon as possible to establish care as a patient. This way, your PCP can learn about you and your medical history before you have a medical problem. This will help you avoid any delays the first time you need to use your benefits.

Before your appointment, write down any questions you may have so you remember to ask them. Also, write down any history of family health problems. Make a list of any prescriptions, over-the-counter drugs and vitamins you take as well.

IF YOU ARE PREGNANT: See your PCP as soon as possible if you are pregnant or if you have a baby less than one year old.

When you become a member of Health Share, you can select a physical health plan and dental health plan from among our health plan partners. If you don’t select a plan, one will be assigned for you. Your mental health plan is determined by the county in which you live.

You may change your PCP as permitted. 
You may change your PCP more often if you move or are discharged from your PCP clinic. 

Call the Customer Service Department at your health plan and they’ll help you find a PCP whose office is convenient for you. They have the most up-to-date information on providers who accept new patients.

Tell the Customer Service Representative that you want to choose a PCP. After you choose a PCP, we will mail you a new Health Share ID card. The card shows the name of the PCP you chose.

Important To Know
When you choose a new PCP, the change is effective right away. However, it might take a few days for your new PCP to get information about you. If you or the PCP’s office staff have questions about your PCP assignment or plan benefits, please call Customer Service.

All children in foster care have special health care needs. If your foster child is assigned to Health Share, our Customer Service Member Navigator can work with you to help you get started with coverage, working with you to ensure access to care, even finding a primary care provider for your foster child. If you are a new member with Health Share and you need to get medical care or prescriptions right away, please call Health Share Customer Service. We will help you.

Your physical, mental and dental health plans are listed on the inside of your Health Share Member ID Card, along with their customer service numbers. Call your plan with questions about your primary care provider assignment or to make an appointment.

To make an appointment for mental health services, call the Mental Health program in the county you live. This is listed on your Member ID Card.

If you need a medical appointment, call your PCP’s office or clinic. 

When you call your PCP for an appointment, be sure to call during the office or clinic’s hours.
Tell the office or clinic that you are a Health Share member
Give them your name and Member Number 
Tell them why you need an appointment

Call in advance for routine appointments that are not emergencies. If you are sick and need to see someone that same day, tell the clinic’s staff person.

Usually you should be able to get a routine or follow-up appointment within one month. Or if you have an urgent issue, you should be able to get an appointment within 48 hours.

If you have questions or concerns about getting a timely appointment, please contact Health Share Customer Service for help.

Perhaps you don’t speak English or you need sign language interpretation. You can have a translator at your appointment, free of charge. Just let your PCP’s office or clinic know you need a translator when you call to make an appointment.

Oregon Health Plan members may be eligible to receive free transportation to their medical appointments. In Multnomah, Clackamas and Washington counties, call Ride To Care at 503-416-3955 (local) or toll free at 1-855-321-4899. You can also use 711 for Oregon Relay Service or TTY/TDD.

If you are a new member with Health Share and you need to get medical care or prescriptions right away, please call Health Share Customer Service. We will help you. It is important to continue to get any ongoing care you need. So make an appointment with your PCP right away.

Once you are assigned to a dental plan, you will need to choose a clinic or dental office as your Primary Care Dentist (PCD). Please call the Customer Service number next to the Dental Plan on your Health Share Member ID card. Your plan can help you find a dentist in your area or make sure you stay with a dentist you have already been seeing.

Your dentist will work with you to take care of your dental needs.  We encourage you to make an appointment soon after you choose a dentist. Don't wait until you have a dental emergency to begin seeing him/her.

If you think you need to see a specialist or other provider, make an appointment with your PCP first. Your PCP will decide which services and tests you may need. If you need to see a specialist or other provider, your PCP will refer you. However, you can see specialists for some kinds of care without seeing your PCP first. You can read more about this in your Health Share member handbook, which will come in the mail.

Health Share has a large network of over 16,000 providers. When you become a Health Share member, you can choose a Primary Care Provider from your health plan’s provider directory or we can assign one for you. You can find all of our health plans’ provider directories on this website by health plan under My Health Plan or at Provider Directories.

Your PCP’s name and phone number is listed on the inside of your Health Share Member ID Card

You will also receive a Health Share Member Handbook in the mail. This will provide you with more information on how to access your benefits. Please take time to look over this handbook when you receive and save it so you can check it later if you have questions. 
You can also find the Member Handbook on this website and in an audio format you can listen to.

When you receive your Health Share Member ID Card in the mail, please detach your ID card from the welcome letter that came with it by using the perforations along the sides of the card. Fold it and keep it with you.

IMPORTANT: Take your Oregon Health ID card and your Health Share ID card to all your health care appointments. Also take your ID cards to the pharmacy when you fill a prescription. Your prescription information is on your ID card.

Providers use the information on your ID cards to check your eligibility. Having your ID cards with you makes checking in for appointments quicker and easier. You also need your ID cards to get medicine at a pharmacy.
